Registration Forms

A form is necessary to register an animal in the database. Therein, the owner consents to have his personal information processed. Pursuant to requirements of the Inspector General for the Protection of Personal Data, the registration form is a necessary tool that facilitates keeping personal data by veterinarians.

The form consists of three parts. The first one is white (the original), given to the owner after the procedure. Two remaining pink copies stay at the vet's or are archived in SAFE-ANIMAL headquarters.
